  1. Any cancellation of scheduled treatment must be given with a 24-hour notice, unless medical or other emergency necessitates. Significant tardiness of 30 minutes or more will result in loss of scheduled time slot and / or a no call fee of $100.00. It is understood that weather may be an unavoidable circumstance.
  2. Patients are to arrive 15 minutes prior to scheduled treatment time to allow for organization of necessary precursors to the dive. (eg. Bathroom, diaper changes, change of clothing etc.)
  3. Please do not bring any valuables with you, as we are not responsible for any lost or stolen items. A locker is provided for storage of your change of clothes, car keys etc.
  4. All payments must be received before treatments are rendered, unless other arrangements have been made prior to treatment with the Office Manager. A block of payments is preferred to allow for advanced scheduling.
  5. There is an initial consultation fee of $100.00 with one of our staff Physicians of which a comprehensive history and physical will be done to determine the need for HBOT.
  6. You will need a prescription with diagnosis from your referring Physician, a recent chest x-ray no more than 30 days old, insurance cards and identification present for your initial evaluation.
